Please find enclosed a questionaire regarding information about your health and possible contraindications for a sound bath; some questions target specifically 1 2 1-sessions (putting bowls or tuning forks on to the body, which won't happen during a sound bath).
Could you please fill these out, they are required for insurance.
As I am also a "Healing practitioner" (Heilpraktikerin), I am liable to the laws for doctors as well, which means medical confidentiality -
all your data stays with me in a safe place and won't be disclosed to anyone.
If you have problems printing out the template - no worries: I have the forms anyway and you can fill it in by arrival.
